Total knee replacement (TKR) is one of the most common major orthopaedic procedures in Johor. The surgery solves the structural problem.

What you do for the 3–6 months after it determines whether you end up with a normal, pain-free, well-functioning knee - or a stiff, painful one that never quite works right.

The four phases

Phase 1 (week 1): Safe transfers, walking with frame or crutches, aggressive pursuit of full extension (straightening). Losing full extension in these early weeks is the single biggest predictor of poor long-term outcome.

Phase 2 (weeks 2–6): Restore bend to 0–90°, normalise gait, reduce swelling, begin loaded strength work.

Phase 3 (weeks 6–12): Reach 110–120° bend, walking without aid, stairs one at a time, progression to two-at-a-time, driving return around week 6–8 if the surgeon allows.

Phase 4 (months 3–6): Normal daily life, cycling, swimming, gradual return to light recreational activity.

Cost

RM120-250 per session. Home-visit recommended for the first 4–6 weeks when mobility is limited (RM120-250). Full course: 20–30 sessions over 3–6 months.

When can I drive again?
Typically 6–8 weeks post-op once you can reliably perform an emergency stop, confirmed by your surgeon.
Will I ever fully bend my knee?
Most patients reach 115–125° bend by month 3–6. Full pre-op bend is not always regained; function is the goal, not degrees.
How soon will I feel a difference?
Most patients feel some change within the first 2 to 3 sessions. Bigger functional gains usually arrive between weeks 3 and 6.
Do I need a doctor's referral first?
No referral is required to see a physiotherapist in Malaysia. We will refer back to a doctor if a red flag turns up.
Can my family insurance cover it?
