Manual therapy covers the range of hands-on techniques physios use - joint mobilisation, manipulation, soft-tissue release, neural mobilisation. It's most effective when paired with specific exercise therapy - not as a stand-alone 'fix'.

Manual therapy is the umbrella term for all the hands-on techniques a physiotherapist uses - joint mobilisation, manipulation (the "click" techniques), soft-tissue release, trigger-point work, neural mobilisation, and fascial techniques.

What matters is not any single technique but how it's used: to create a window of reduced pain and better movement that the patient then fills with the right exercises.

What it treats well

  • Mechanical low back and neck pain.
  • Stiff joints after prolonged immobilisation.
  • Post-surgical range-of-motion restoration.
  • Shoulder stiffness (frozen shoulder, rotator cuff tendinopathy).
  • Hip and knee stiffness in early osteoarthritis.
  • Trigger points and muscle guarding.

What it does NOT do

  • Fix the underlying cause of chronic pain on its own.
  • Replace the need for exercise and loading.
  • "Realign" your spine or fix asymmetries - these framings are not clinically supported.

Is it safe for neck pain?
Yes for most people. Physios screen for red flags (vertebrobasilar signs, upper cervical instability) before high-velocity neck manipulation.
