Shoulder Labral Tear (SLAP / Bankart)

A torn labrum can be managed conservatively when the shoulder is stable and the tear pattern is suitable. Rehab focuses on rotator cuff and scapular control over 12–20 weeks.

The labrum is a ring of cartilage that deepens the shoulder socket and helps the humeral head stay centred.

Tears are named by location: SLAP (superior, involving the biceps anchor), Bankart (anteroinferior, classic after a dislocation), posterior (often from repetitive pushing - gym bench press a common culprit), and degenerative (usually with rotator cuff pathology in older shoulders).

Surgery vs rehab - a more nuanced decision

A labral tear on MRI does not automatically mean surgery. Plenty of 40-year-old shoulders have asymptomatic SLAP tears.

The decision depends on: mechanism of injury, whether the shoulder has actually dislocated, how unstable it feels day to day, and what sport or job you need it for.

Most first-time anterior dislocations in patients over 25 can be trialled on rehab first; recurrent dislocators and younger contact-sport athletes often end up with Bankart repair.

What rehab involves

Early: pain modulation, graded range-of-motion, activation of rotator cuff and scapular stabilisers without provoking the tear.

Mid-phase (weeks 4–10): progressive strengthening with attention to the direction the tear is most vulnerable in (avoid deep external rotation with abduction early on for Bankart).

Late phase (weeks 10–20): sport-specific - throwing mechanics for cricketers and javelin athletes, overhead pressing for gym-goers, swim stroke for pool athletes.

Cost and timeline

RM120-250 per session. Conservative path: 12–20 sessions over 16–20 weeks. Post-operative path (Bankart or SLAP repair): usually starts at 6 weeks post-op and runs 16–24 weeks to return-to-sport testing.

FAQs

Can a labral tear heal without surgery?
The labrum itself doesn't heal like a muscle, but many tears become symptom-free as the surrounding cuff and scapular system learn to compensate. We trial 12 weeks of structured rehab first; overhead athletes who fail that may go to surgical consult.
If I go for surgery, how does rehab change?
Post-op follows the surgeon's protocol - usually a sling for 4–6 weeks, then a tightly paced loading progression. Returning to overhead sport is typically at month 5–6 and takes structured physio to get there safely.
How soon will I feel a difference?
Most patients feel some change within the first 2 to 3 sessions. Bigger functional gains usually arrive between weeks 3 and 6.
Do I need a doctor's referral first?
No referral is required to see a physiotherapist in Malaysia. We will refer back to a doctor if a red flag turns up.
Can my family insurance cover it?
