Frozen Shoulder Physiotherapy in Johor

Frozen shoulder (adhesive capsulitis) is one of the most frustrating conditions we see.

It starts with what feels like a minor shoulder niggle, becomes progressively painful, and then stiffens to the point where combing your hair or reaching into your back pocket is almost impossible.

The literature says it usually resolves on its own in 12–18 months.

That is true - but what the literature doesn't capture is how much function people lose permanently when they tough it out without treatment.

The three stages - and why they need different physio

Freezing (weeks 1–12): progressive pain, especially at night, with gradually reducing range. Goal: control pain, gentle mobilisation, maintain what range remains.

Aggressive stretching at this stage makes things worse.

Frozen (months 3–9): pain eases but stiffness dominates. Goal: active mobilisation, manual therapy, progressive stretching into the now-tolerable range.

Thawing (months 9–18+): gradual return of movement. Goal: load progression, strength, return to full functional use.

A physio who does not know this staging will either over-push in the painful phase or under-push in the stiff phase.

Timeline expectations

Is physio right for you?

  • Shoulder pain and stiffness for 2+ weeks, progressively worse.

Johor-specific considerations

Diabetic patients, especially those newly diagnosed or with suboptimal HbA1c, see frozen shoulder more often and more severely. We often coordinate with your endocrinologist or GP - Johor has good diabetes-aware physios in Skudai, Iskandar Puteri, and Kluang.

When frozen shoulder is NOT the diagnosis

Rapid onset after trauma, weakness when lifting the arm, catching/locking sensation - these suggest rotator cuff or labral pathology instead.

FAQs

Will frozen shoulder go away on its own?
Eventually, yes - typically 12 to 18 months - but you can lose significant range of motion along the way that is hard to fully regain. Staged physiotherapy shortens the course and preserves movement.
Is an injection needed?
Corticosteroid injection can be very helpful in the painful first stage when used alongside physio. Discuss with your doctor; we can liaise with an orthopaedic specialist if one is needed.
Is frozen shoulder linked to diabetes?
Yes - diabetics have a 3–5× higher lifetime risk. Blood sugar control during the frozen phase is particularly important.
How soon will I feel a difference?
Most patients feel some change within the first 2 to 3 sessions. Bigger functional gains usually arrive between weeks 3 and 6.
Do I need a doctor's referral first?
