Meniscus Tear

Meniscus tears fall into two buckets: traumatic (sports, twisting injury) and degenerative (age-related wear). Degenerative tears usually respond to physio alone. Traumatic tears with mechanical locking often need surgery.

The meniscus is a crescent-shaped piece of cartilage in the knee that acts as a shock absorber.

Tears come in two flavours: traumatic (sudden twist in sport, fall, football tackle) and degenerative (gradual wear with age, often seen on MRI in 50s–60s).

Traumatic vs degenerative - why it matters

Traumatic tears in younger active patients, especially with mechanical locking, giving-way, or inability to fully straighten the knee, may need arthroscopic surgery. Post-op rehab runs 6–8 weeks.

Degenerative tears - the ones incidentally found on MRI in middle-aged and older patients - respond remarkably well to physio alone. Current best evidence favours 8–12 weeks of structured exercise therapy before considering surgery for this group.

  • Quadriceps and hip strength deficits.
  • Flexibility and range of motion.
  • Load management - gradual return to activity.
  • Neuromuscular control - preventing the giving-way pattern.

FAQs

MRI shows a meniscus tear - do I need surgery?
Not automatically. Many degenerative tears improve with physio; surgery is mainly for traumatic tears with mechanical symptoms (locking, giving way).
How soon will I feel a difference?
Most patients feel some change within the first 2 to 3 sessions. Bigger functional gains usually arrive between weeks 3 and 6.
Do I need a doctor's referral first?
No referral is required to see a physiotherapist in Malaysia. We will refer back to a doctor if a red flag turns up.
Can my family insurance cover it?
Most private medical plans in Malaysia cover physio with a doctor's note. We can WhatsApp you a session brief to attach to a claim.
Will the same physio see me each session?
