Turf Toe Physiotherapy in Johor
Turf toe is a sprain of the big toe's metatarsophalangeal joint, caused when the toe is forcibly bent upward while the ball of the foot is planted.
Classic mechanisms: a footballer's foot caught under a tackle on hard astroturf, a rugby lineman pushing forward, a dancer's forced toe dorsiflexion, or a pickleball player lunging with the front foot sticking as the body rotates forward.
Grading and what it means in weeks
Grade 1: joint capsule stretched, local soreness, full weight-bearing possible. 1–3 weeks. Grade 2: partial capsule tear, moderate swelling and bruising, limp obvious. 3–6 weeks. Grade 3: complete capsule tear (sometimes with sesamoid involvement), severe swelling, can't bear weight normally. 8–12+ weeks. Grade 3 with sesamoid fracture may require imaging and surgical consultation.
Rehab overview
Early: rest from running, turf toe taping to block extreme dorsiflexion, stiff-soled shoe or carbon plate insert, manual mobilisation to restore pain-free range.
Mid: progressive calf and intrinsic foot loading, return-to-jog after painless walking is established.
Cost and Johor context
RM120-250 per session. 4–10 sessions across 4–10 weeks for most presentations. We see this regularly in Stadium Larkin footballers, semi-pro rugby players, and - the new entrant - pickleball converts whose forward lunges exceed what their toe capsule has ever had to take.
