Plantar Fasciitis Physiotherapy in Johor

What drives plantar fasciitis

Contrary to the name, it's rarely an inflammation.

It's a load-tolerance problem: the plantar fascia (a thick band of connective tissue running from heel to toe) has been asked to do more than its current capacity.

Physiotherapy fixes it by progressively building that capacity back up - not by resting until it magically recovers.

Timeline expectations

  • Weeks 1–2: Modest pain reduction, foot and calf education.
  • Weeks 3–6: Progressive loading of the calf complex and plantar fascia - high-load heel raises are the workhorse.
  • Weeks 7–10: Return to most daily activities, running reintroduction in runners.
  • Weeks 10–14: Full return to activity with maintenance loading.

What a first session looks like

History (pain pattern, footwear, activity, weight, past injuries), examination (palpation, dorsiflexion range, windlass test, calf length, foot posture), gait analysis if a runner, and initial exercise prescription.

Home programme: calf raises 3×/week at escalating load, paired with gentle mobility.

FAQs

Do I need arch supports or custom orthotics?
Off-the-shelf supports help some patients in the acute phase. Custom orthotics are rarely necessary - the bulk of recovery comes from exercise, not inserts.
Does shockwave therapy actually work?
For chronic (> 3 months) plantar fasciitis that hasn't responded to exercise alone, yes - evidence is moderate to strong. We use it selectively.
Should I stop running?
Not always. Modified running volume + rehab works for many. Complete cessation often makes the tissue deconditioned and slower to recover.
How soon will I feel a difference?
Most patients feel some change within the first 2 to 3 sessions. Bigger functional gains usually arrive between weeks 3 and 6.
Do I need a doctor's referral first?
