Sciatica

Sciatica is leg pain radiating from the lower back, usually caused by a nerve root being irritated or compressed. Most cases settle with 6–10 sessions of physiotherapy that combines nerve-calming techniques, specific exercises, and a graded return to activity.

Sciatica Physiotherapy in Johor

Sciatica is the label we put on any pain, tingling, or weakness that follows the path of the sciatic nerve - from the lower back, through the buttock, down the back or side of the leg, sometimes as far as the foot.

It is not a diagnosis in itself.

It is a symptom of something irritating the nerve - most commonly a disc bulge, less commonly piriformis tightness, spinal stenosis, or referred pain from the hip.

What drives sciatica, clinically

In most Johor cases we see (office workers in JB city and Iskandar Puteri, port and factory workers in Pasir Gudang, long-distance drivers, older patients with early spinal stenosis), a disc in the lower lumbar spine is the source.

The disc either bulges mechanically, releases an inflammatory signal, or compresses a nerve root - often all three to some degree.

Physiotherapy targets the mechanical component (positions and movements that offload the nerve), the inflammatory component (load management, graded exposure), and the strength/control deficits that allowed the problem to develop in the first place.

How long does recovery take?

Acute sciatica with clear mechanical pattern: most improve significantly within 3–4 sessions and return to normal activity by week 6–8.

Longer-standing cases with bilateral or stenotic features take 10–12 weeks or more.

Full recovery is measured not just by pain reduction but by return to sitting, driving, and working without flares.

Is physio right for you?

  • Pain radiating below the knee, tingling, or intermittent leg weakness.
  • No red flags: saddle anaesthesia, bladder/bowel changes, progressive bilateral leg weakness, recent cancer history.
  • Symptoms for more than a week, or recurring.
  • You are willing to do a daily exercise programme - this is essential for durable recovery.

What happens at the first session

Detailed history and red-flag screening, neurological tests (straight leg raise, slump, reflexes, myotomes, sensation), classification (directional preference - does the pain ease with extension, flexion, or neither?), initial treatment, and a specific home programme.

You should feel a measurable improvement in one of your movement tests by session 2.

Johor-specific considerations

Pasir Gudang / industrial lifting injuries - SOCSO panel clinics will manage these if filed within 60 days; we match accordingly.

Singaporean long-distance commuters - frequent drivers get sciatica flares from prolonged sitting. We plan session cadence around their crossing schedule.

When sciatica needs immediate medical attention

  • Saddle anaesthesia (numbness between the legs like sitting on a saddle).

FAQs

How do I know it's sciatica and not just back pain?
Sciatica pain typically travels below the knee and often feels burning, electric, or like pins and needles. If the pain stops at the buttock or upper thigh, it's usually not true sciatica.
Do I need surgery for sciatica?
Most sciatica - even with disc involvement - resolves without surgery. Surgery is considered only if there's severe weakness, bladder/bowel changes, or 6–8 weeks of good physio hasn't helped.
Will bed rest help?
No - prolonged bed rest makes sciatica worse. Structured movement with a physio's guidance is the evidence-based approach.
How soon will I feel a difference?
Most patients feel some change within the first 2 to 3 sessions. Bigger functional gains usually arrive between weeks 3 and 6.
Do I need a doctor's referral first?
No referral is required to see a physiotherapist in Malaysia. We will refer back to a doctor if a red flag turns up.
