Golfer's Elbow (Medial Epicondylitis)

Pain on the inside of the elbow, triggered by repeated gripping - mechanics, kitchen workers, desk users clenching a mouse. Eccentric wrist-flexor loading over 8–12 weeks is the proven fix.

Medial epicondylitis - "golfer's elbow" - is pain on the inside of the elbow, roughly where the bony point sits.

The name is misleading. In a Johor clinic, barely one in ten cases actually plays golf.

The real drivers are repeated forceful gripping: mechanics at Pasir Gudang yards, F&B workers at Mount Austin food courts wringing cloths all shift, forklift drivers, and office workers who squeeze a mouse like it owes them rent.

How it differs from tennis elbow

Tennis elbow is on the outside of the elbow; golfer's elbow is on the inside.

Tennis elbow involves the wrist extensors (the muscles that pull your wrist back); golfer's elbow involves the wrist flexors and pronators (the muscles that curl your wrist forward and twist it palm-down).

Treatments look similar at a glance but the specific loading is opposite.

What rehab actually involves

A first session identifies the load that's exceeding the tendon's tolerance, modifies your grip mechanics at work, and introduces isometric wrist-flexor holds.

From week two, we load with slow eccentric wrist flexion using a dumbbell or a resistance band.

Manual therapy and dry needling can help early pain, but they don't fix the tendon - loading does.

Cost and timeline

RM120-250 per session. Expect 6–10 sessions over 8–12 weeks. Chronic cases (symptoms longer than six months) may add shockwave in weeks 4–8.

Is it right for you?

  • Sharp pain on the inside of the elbow when you grip, lift a kettle, shake hands firmly.
  • Tenderness right on the bony bump on the inside of the elbow.
  • Pain worsens with wrist curls or when you twist a screwdriver.
  • No numbness or tingling in the ring and little fingers (that points to ulnar nerve, different problem).

Do I need to stop training completely to fix golfer's elbow?
In most cases no - we modify load rather than stop it. Heavy gripping drops for 2–3 weeks while we rebuild wrist-flexor tolerance with slow, heavy eccentrics. Most patients in our Johor caseload return to full training at 6–10 weeks.
Is it the same as tennis elbow?
Same pattern (tendinopathy), different tendon. Golfer's elbow affects the inside of the elbow (wrist flexors); tennis elbow the outside (wrist extensors). Rehab logic is similar but targets the opposite forearm group.
How soon will I feel a difference?
Most patients feel some change within the first 2 to 3 sessions. Bigger functional gains usually arrive between weeks 3 and 6.
Do I need a doctor's referral first?
No referral is required to see a physiotherapist in Malaysia. We will refer back to a doctor if a red flag turns up.
Can my family insurance cover it?
