When the JB transition makes sense

Good fit:

  • Patient medically stable, past the acute hospital phase (typically week 3+).
  • Family has a base in JB (home or extended family) so the patient can stay rather than cross per session.
  • Core impairments are musculoskeletal and motor (weakness, gait, balance, upper-limb function) - these translate well to JB rehab.
  • Family has a willing caregiver who can participate in sessions and deliver home programme daily.

Not a fit:

  • Patient still medically unstable or on intensive monitoring.
  • Complex swallowing or speech issues requiring specialised speech therapy that is thinner in JB than SG.
  • Patient can't be safely transported across the checkpoint.
  • No family base in JB - daily crossing for stroke rehab is exhausting and usually not sustainable.

The pathway we structure

Weeks 0–3 (in SG): Stay in the SG system. Inpatient + early outpatient rehab in SG, where the multi-disciplinary team (OT, speech, neuro physio, case manager) is concentrated and free or subsidised through public channels.

Weeks 4–24 (JB-based, with SG check-ins):

  • Base in JB if family has a home there.

Weekly neuro-physio sessions - JB has strong Mandarin- and English-speaking neuro physios in Mount Austin, Iskandar Puteri, and Taman Molek.

  • Monthly check-in back in SG with the original rehab team, to calibrate goals and catch anything that needs escalation.
  • Daily family-led home programme.

Caregiver training is non-negotiable for this to work.

Months 7–12+ (maintenance):

  • Reduce clinic frequency - typically one session a week drops to fortnightly, then monthly.
  • Family-led programme becomes the core of day-to-day rehab.
