Singapore surgery, JB rehab - how the handover actually works

Many Singaporeans have their surgery in Singapore but prefer JB for post-op rehab. Here's how we structure the handover - operative notes, protocols, scheduling around CIQ, and what's covered by which policy.

A pattern has become common over the last few years: Singaporeans whose health is covered by a Singapore hospital for the acute operative phase, then cross to JB for the months of post-operative rehabilitation.

Done well, this saves 50–70% on the rehab bill versus staying in Singapore, with clinical quality that many patients rate as equivalent.

Done badly, you lose the operative surgeon's protocol in translation. Here's how we handle the handover.

Step 1 - Get the operative note

Before the first JB session, we need the operative note or a letter from the Singapore surgeon.

TTSH, SGH, NUH, Mount Elizabeth, Gleneagles Singapore and Raffles Hospital all produce these as standard. Patients can usually request them directly through the hospital's medical records department.

Without this, we are guessing - and we don't guess on post-op patients.

Step 2 - Confirm the surgeon's specific protocol

Singapore surgeons vary in their post-op preferences the same way Johor surgeons do. Some use four-phase ACL protocols; others use a different split.

Some want a sling for 4 weeks post-rotator cuff; others go to 6.

We respect the operating surgeon's letter to the week - even when a Johor-based approach would do it slightly differently - because they are the ones who held the tissue during surgery.

Step 3 - Schedule around CIQ realities

A 60-minute physio session can mean a 4-hour day with customs.

We schedule post-op patients in mid-morning or mid-afternoon slots, ideally with flexibility if traffic is bad.

Some patients book a block of 3–4 sessions across a full day in JB - see us morning, rest or eat, see us again afternoon.

Others stay overnight in JB for the first few critical weeks of rehab.

Step 4 - Insurance paperwork done properly

Integrated Shield plans, MediSave-linked integrated policies, and private-top-up insurance all have their own rules on overseas physio coverage.

We provide clinical notes in the format insurers expect; patients confirm coverage with their own insurer in writing before treatment starts.

What conditions this works well for

  • Post-ACL reconstruction.
  • Post-knee and hip replacement.
  • Post-rotator cuff repair.
  • Post-spine surgery (microdiscectomy, laminectomy, fusion).
  • Post-fracture fixation.
  • Complex chronic pain programmes where a longer, more intensive course than Singapore physio commonly provides is valuable.
