This case follows a 6-month post-stroke rehab for a 74-year-old grandmother (we'll call her Mdm Lim) who had her stroke in Singapore, was discharged from a SG public hospital, and chose to do her rehab from her son's home in Skudai, Johor.

The decision: SG home-visit vs JB home-visit
Mdm Lim was a left-MCA stroke with right-sided weakness and mild expressive aphasia.
SG home-visit physio at the discharge price the family was quoted was SGD 250–300 per visit.
The family also lived in a 4-room HDB with limited space for transfer training.
The son had a second home in a Skudai single-storey landed property - easier transfers, garden access, and crucially, his father (Mdm Lim's husband, who would be the primary at-home coach) lived there.
The family asked us if a 6-month JB rehab via Woodlands made clinical sense.
Why dialect mattered
Mdm Lim is Hokkien-primary at home. Mandarin is her second language.
English is functional but tires her quickly post-stroke.
The SG physio they had been quoted for would have worked.
Match decision
We matched a Hokkien-fluent neuro physio based in Skudai who works with stroke patients across western JB.
Two visits per week for the first 3 months, then weekly.
Sessions ran 60 minutes - 35 minutes hands-on, 25 minutes coaching the husband on between-visit drills.

- SG home-visit equivalent (36 visits × SGD 275) = SGD 9,900.
- Net saving: ~SGD 7,020. About 71% lower at this cadence.
- Causeway costs: minimal - physio drove to the home; no causeway crossing involved for the patient.
Why the model worked
The SG family wasn't escaping clinical care quality - they were buying back home-visit cadence and dialect fit at sustainable cost.
The first 6 months post-stroke is the highest-recovery window; under-dosing rehab there has long consequences.
Caveats
- This pathway works only if you have viable accommodation in JB for the rehab period. Mdm Lim's situation was unusual.
- Medication management remained on the SG side (Mdm Lim travelled back monthly for SG GP follow-up).
- Acute medical complications would have triggered a same-day SG transfer.
