Elbow complaints split cleanly into a handful of patterns.
Most are overuse tendon problems - tennis elbow on the outside, golfer's elbow on the inside - and in our Johor population, both are driven more by gripping load at work or on a pickleball paddle than by actual racquet sports.
The rest is post-fracture stiffness, nerve entrapment (cubital tunnel for the ulnar nerve, radial tunnel less often), and occasional post-traumatic joint issues.
Why loading direction is the whole game
Tennis elbow involves the wrist extensors. Golfer's elbow involves the wrist flexors and pronators.
Loading one when the patient has the other flares the problem and wastes weeks.
Assessment confirms the pattern with specific tests (Cozen's and Mill's for lateral; reverse tests for medial) and a proper palpation of the exact epicondyle involved.
Other elbow patterns we treat
- Post-fracture stiffness - radial head, olecranon, or supracondylar fractures after cast removal.
- Ulnar nerve irritation - numbness in ring and little finger, often worse at night with a bent elbow.
- Distal biceps tendinopathy - a less-common but under-recognised cause of anterior elbow pain in weightlifters.
- Pitcher's / thrower's elbow - in cricketers and overhead athletes.
