The honest working range across the state is RM120-250 per session, with most patients paying RM120-250 for a standard musculoskeletal case.
The range reflects three things: clinic tier, session length, and location. Below is what we actually see across our Johor referral network.
The range, broken down
RM120-250 - the most common range. Mid-tier private clinics in Skudai, Kulai, Johor Jaya, Taman Molek, Permas Jaya, Tampoi, Kluang town, Batu Pahat town, Muar town. 45-minute sessions, standard manual therapy + exercise prescription.
RM120-250 - the broad middle. Bigger or newer clinics in JB city centre, Mount Austin, Austin Heights, Pulai, Senai area. Often 60-minute sessions, may include adjuncts like shockwave or dry needling at no extra charge.
RM120-250 - the top end. Iskandar Puteri / Medini hospital-adjacent private clinics - near Gleneagles Medini, Columbia Asia Iskandar, Mid Valley Southkey.
Longer sessions, premium facilities, often with imaging-adjacent services. Also includes specialist sports physiotherapy practices in JB.
Home-visit physio - RM120-250. Travel distance is the biggest variable. A home visit in JB city core is cheaper than one to Ulu Tiram or Gelang Patah.
What actually drives the price
Three factors in order of importance: time per session, physio seniority, and adjunct services included.
A 60-minute session with a senior physio who owns the clinic and uses shockwave is meaningfully different from a 30-minute session with a junior running parallel patients.
The rate reflects that.
Equipment branding matters less than many clinics imply.
A Storz Medical shockwave machine and a cheaper Chinese equivalent both do the job for most indications - what matters is the physio's protocol and dosing, not the logo on the device.
SOCSO and insurance
SOCSO panel physios charge the SOCSO-set fee for eligible workers (typically factory, port, construction injuries).
You do not pay out of pocket if the claim is approved.
The panel list is available on the SOCSO website; Your SOCSO panel doctor refers you to a panel physio if your case qualifies.
Private medical insurance - most Malaysian plans cover outpatient physiotherapy with a doctor's referral up to an annual cap.
Check your panel list and cap. Singapore integrated shield plans generally do not cover overseas physio.
The Singapore cross-border picture
Singapore private physio typically runs SGD 120 to SGD 180 per session (SGD 150–250 for sports specialists).
At a reference SGD/MYR rate of 3.5, a SGD 140 Singapore session equals RM 490 - vs RM 180 for a comparable JB session.
For an 8-session course you are looking at roughly RM 2,400 vs RM 4,400 SGD equivalent, before travel.

What a full course typically totals
Most mechanical musculoskeletal cases resolve within 4 to 8 sessions.
Physio pricing is shown as RM120-250 per session; total spend depends on the number of sessions needed.
Post-surgery, neurological or chronic cases run longer - RM 1,500 to RM 3,000 over the first 3 months is a realistic range.
Red flags on pricing
- Any "wellness centre" charging RM 80 per "physio" session without showing MOH registration - that is not physio, regardless of what the banner says.
- Clinics demanding upfront packages of 20 sessions. Good clinics let you pay per session and recommend the realistic number only after assessment.
- Steep "opening promotion" rates - fine as a trial, but the real ongoing rate is what you should budget for.
