Physiotherapy vs Chiropractic - What's the Difference?
Physiotherapy and chiropractic both treat back and neck pain, share some manual therapy techniques, and both exist as regulated professions.
Patients often ask which to choose.
The short answer is: they have overlapping scope but different training, different emphasis, and different evidence profiles.
Training and scope
Physiotherapists complete a 4-year Bachelor of Physiotherapy (BPhysio) covering neurology, cardiorespiratory, musculoskeletal, paediatric and women's health, post-surgical rehab, and sports. Registered under the Allied Health Professions Act 2016.
Chiropractors complete chiropractic training (typically 4–5 years in an accredited programme, often overseas).
Scope of practice is focused on spinal and musculoskeletal manual therapy, particularly adjustments (high-velocity manipulation).
Registered under a separate Malaysian regulatory framework.
Philosophy and approach
Physiotherapy combines manual therapy with active rehab - exercise is almost always the larger share of the plan.
Chiropractic has historically placed more emphasis on manipulation as a primary treatment, though modern chiropractic has evolved and many chiropractors now integrate exercise.
Individual practice varies.
Where evidence lines up
- Acute low back pain: both can help short-term.
- Chronic low back pain: combined active rehab (stronger physio emphasis) outperforms manipulation-only.
- Neck pain: similar.
- Post-surgical rehab: physio is the default.
- Complex neurology, paediatrics, pelvic health: physio domain.
- Sports rehab with return-to-sport testing: physio domain.
How to choose
- Want hands-on work plus a strong active rehab plan you take home? Physiotherapy.
- Prefer spinal adjustments as a primary treatment? Chiropractic.
- Want insurance or SOCSO coverage? Check the specific policy - many cover physiotherapy only.
