Thoracic Spine (Mid-Back)

Often the missing piece in stubborn neck and shoulder cases. Also the home of thoracic disc issues and rib-joint pain. Assessment and manipulation deliver fast change when correctly indicated.

Thoracic Spine Physiotherapy in Johor

The thoracic spine - the 12 vertebrae between the neck and the lumbar spine, each attached to a rib - is often the silent driver of neck, shoulder and headache complaints.

A stiff thoracic spine forces the neck to extend excessively to see a screen and forces the shoulder to find overhead range somewhere else.

Direct thoracic pain (people pointing between the shoulder blades) is less common but real.

Where it sits in the bigger picture

Most of our thoracic work is not for "thoracic pain" as a primary complaint.

It's for:

  • Stubborn rotator cuff or shoulder impingement cases where overhead range won't unlock.
  • Chronic neck pain that stays stuck despite neck-specific treatment.
  • Tension headaches linked to poor mid-back mobility.
  • Postural cases in long-hour Iskandar Puteri and Medini desk workers.

When patients present with true thoracic pain, we screen more carefully - for rib joint dysfunction (costovertebral), thoracic disc (uncommon but serious), or referred visceral pain.

What the treatment involves

Mobilisation of stiff segments, occasional manipulation (HVLA thrust) where clearly indicated and screened, thoracic rotation and extension drills, strengthening the scapular stabilisers and deep spinal erectors.

The result is a mid-back that rotates freely, which in turn lets the neck and shoulder do their jobs.

Cost and Johor context

RM120-250 per session. Runs 6–10 sessions as part of a combined plan, usually alongside shoulder or neck rehab.
